Appointments & Consultations

Please call our office to schedule an appointment.  We see patients by appointment. In case of emergency we do accept same day appointment with a doctor referral.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule.

What to Bring to Your Appointment

1. Your medical insurance card(s) and driver license
2. A list of current prescription or over-the-counter medications you are taking, including dosage and frequency
3. Pertinent information about your medical and surgical history (if applies)
4. Any recent x-rays or appropriate records you may have

Cancellations

As a courtesy we will attempt to remind you of your scheduled appointment time, but it is the responsibility of the patient to remember to keep the appointment.
Should you need to cancel or reschedule your appointment, we would greatly appreciate if you could notify our office at least 24 hours before your scheduled appointment time.  Thank you for your consideration.

Insurance Accepted:

Please check with your insurance carrier to determine your benefits. Our office can provide you with other specific information. All co-payments, co-insurances, deductibles and past due balances are due at the time of your appointment.

If your insurance plan is not listed here, please don’t hesitate to call our office, we’ll be happy to help you with any question or concern you might have. We might also be able to give you a discounted rate comparable to the contracted rate.

As you know, your insurance coverage is a personal matter between you, your employer and the insurance company. Your benefits are determined by the type of plan chosen by you and your employer.

We strongly suggest that you directly contact either your employers Human Resources Department or your Insurance Provider to become familiar with the specific provisions of your insurance coverage plan.

We cannot be responsible for knowing your benefit coverage within your individual plan, due to the growing number of insurance companies and underlying plans within each of these companies.

We will be happy to answer any questions you may have but we are not responsible for collections from your insurance company as we are not a party to your insurance contract. Your cooperation and understanding in this matter is greatly appreciated.
